Steps that protect you: (1) Submit the request in writing — email is fine — with specific words 'I am requesting a reasonable accommodation under the ADA for a medical condition.' That magic-words framing triggers the process. (2) Get a one-page note from your provider stating you have a covered condition and what accommodation is requested (don't disclose the diagnosis to HR — your provider's note is enough).
