Final judgment package keeps getting kicked back

Third time the clerk rejected it. Once for the wrong form revision date. Twice for missing signatures on a page I swear was signed. Anyone else been through this?

I had it kicked back four times. The fifth time I went in person to the self-help center and they pointed at every single thing in 20 minutes. Worth the trip.
